We are looking for Intermediaries that:
The Intermediary Network has identified specific functions that must be conducted by one or more institutions in a local community if convening and connecting activities are to be effective and sustainable.
These strategic functions are:
Intermediaries interested in being considered for Network membership must complete the attached application and letter of commitment.
Applications will be reviewed for approval by a panel of network members.

Please answer the following questions to help us identify the most appropriate organizations for participation. Be as specific as possible in your answers, providing evidence and insight into your work as an intermediary.

simplify, manage, and strengthen the interactions among schools, workplace

2. Primary Intermediary Functions of the Organization: Four strategic intermediary functions are critical to successful, sustainable community efforts to connect work and learning for young people. What functions are currently performed by your organization? Please check ALL that currently apply to your organization:

Please respond to the following 10 questions in a brief narrative.
1. Background: Briefly explain the origins and evolution of the organization's involvement in performing Intermediary functions in your community. Please include your organizational vision and mission statement.
2. Description of Exemplary Activities: Identify an activity (or activities) that illustrates how the organization provides at least two functions listed above in question #2. Be specific. Detail the role of this organization in designing and implementing this activity.
3. Goals/Priorities: Take the previous question/answer a step further and describe the activities that have been prioritized for development or expansion in the coming year.
4. Evidence of Effectiveness: What impact is the activity having? In what ways is the organization contributing to improvements in quality, scale, or sustainability of school-to-career and/or youth development efforts? Be as specific as possible.
5. Evaluation and Measurement: What data elements does the organization collect to inform program development and evaluate intermediary effectiveness? Please be specific.
6. Community Readiness/Vision: What key conditions or realizations brought your community to the conclusion that the development or expansion of an intermediary was needed or desired? In other words, why did you decide to build or enhance your capacity to deliver services through an intermediary structure?
7. Organizational Capacity: Describe your organizations capacity to create or expand on the functions described on the previous page. (e.g. staff and resources allocated, operational structures in place, etc.)
8. Connection to the Local STW Partnership or Youth Council: Are you directly connected to a school-to-work/career local partnership or local youth council? Please describe the relationship between the responding organization and the partnership or council. What role does your organization play in helping to sustain STW activity in the community?
9. Network Participation: What value would participation in the Intermediary Network bring to your organization? What priorities would you suggest we address that would best meet your needs? What would you bring to the network?

Commitment to the Intermediary Network Our organization, , performs intermediary functions that help connect schools with workplaces and other community resources so that young people can combine learning with doing and become better prepared for post-secondary learning and careers. Like other intermediary organizations,
we perform a range of functions that are critical to organizing and sustaining effective local youth-serving systems that bridge the worlds of education and workforce development, including: 1) convening key stakeholders; 2) brokering and/or providing services to employers, educational institutions, young people, and the youth-serving system; 3) measuring outcomes of these efforts; and 4) promoting policies that can sustain effective practices. While we may be stronger in the provision of certain functions rather than others, we are committed
to all four of these priority functions and to expanding our efforts in these four
areas. We are interested in being part of a network of other organizations that realize
the critical importance of staffed local organizations that perform these intermediary
functions in their communities. We will be active members of such a network,
sharing our knowledge and experience, learning from and with others, and creating
a voice for ourselves and others like us in debates about education and workforce
